This adapter gives Motorola radios with M3 connectors the ability to use a much more convenient and simple solution, the M2 3.5mm connector with threading. Once attached to the radio there is very little reason to remove the adapter, simply unscrew the M2 connector of your earpiece from the adapter to detach and screw back in when it's time to use the radio. Some users find this solution more elegant than using a standard M3 earpiece.
